Insurance, Payments, and Referrals
Please bring your insurance card, photo ID, and your referral (if required by your insurance). You must have an active insurance card at each visit or you will be rescheduled.
Copayments are due at the time of your service. Payments can be made by credit card, debit card, or cash. Pavilion Obgyn no longer accepts checks for co-payments.
There is a $25.00 fee for no shows, cancellations, or rescheduled appointments without 24 hour notice. If you are 15 minutes late for your scheduled appointment you may need to be rescheduled. This may also result in a $25.00 fee.
There is a $10.00 fee for completion of medical forms.
Phone Calls and Messages
It is our goal to answer all messages as soon as possible, however, please allow 48 business hours for a return of your call. Please leave the reason for your call, as some messages can be handled quickly by the nurse. Please leave your best contact number at the time of your message.
Prescription Refills
Refills for prescriptions will be called in within 48 hours. Therefore, it is important for you to be aware of when your prescription will run out, particularly for birth control methods. If you are given a prescription at your appointment you must take this new prescription to the pharmacy. You cannot call in a new prescription with your old prescription number. Due to time constraints, prescriptions given to the patients at their visits will not be called in. Please do not page the doctors for prescription refills.
After Hour Calls
An on-call physician is available after hours for emergencies only. They may be reached by calling the office phone number and following the prompts. Please leave a phone number that will be available for at least 15 minutes. Please remove all blocks from your telephone as the doctors do block their personal numbers when returning call. If your phone is blocked this will slow down the response. Please utilize this service in the case of emergencies only.
Laboratory Services
There is a phlebotomist available for blood draws in our office. These services are provided from Labcorp only. It is the patient’s responsibility to assure that their insurance will cover labs drawn from this lab. Insurance companies often change their preferred laboratory therefore please check your coverage prior to each blood draw. These services are provided for the convenience of our patients and labs requested from our doctors only. There is an additional full service lab located on the second floor of the Sajak Pavilion. If you are an employee of Anne Arundel Medical Center please inform the staff prior to any blood draws.
Test Results
It is our policy to inform patients of abnormal results in a timely fashion. Please inform the office of any changes in your contact information. Remember, most tests take a week or more for results. Pap smear results are generally not available for at least two weeks.
HIPAA Laws
Patient privacy is very important to us. We cannot discuss your medical information with anyone else unless you list their name on your registration form. We will not release your medical information without your written consent with the exception of emergency or urgent care. For example, we will release your information to another physician who is participating in your care if needed for continuity of care. If you would like a copy of our privacy practices please ask the receptionist.
